Why do some ladies have multiple profiles
Surely it affects their ratings negatively if punters rate on their different profile s

I'm not sure as to which profiles you are referring to, but one reason could be that some ladies offer different treatments on their different profiles.
Some other profiles are also deleted profiles.

Lee in Grayson has 3
Melina has 2
Karin has 2
Don't make sense to me

There are a stack og reasons.

Marketing: Diggerent guys are attracted to diggerent things in progiles. Some want tits n' ass in all their glory, some want subtle sexiness.

Diggerent services: A lady may have diggerent progiles gor massage, GH, BDSM etc.

Areas: some ladies like to emphasise that they service diggerent geographic areas.

Multiple ads in same area don't bother me. What bothers me is when ladies have ads in multiple areas when they actually only operate from one area.

For example, a lady who advertises in both Midrand and Centurion, when they are in Centurion. When you call them, they'll say "well it's near Midrand". Depending on where about Midrand the punter is, and where about Centurion the WG is, it might not necessarily be "near". Sandton or East Rand might be nearer for the punter.

Well, in fairness they are paying for each ad - so just have to accept it.

I work with a lady who has 3 different profiles and you would be surprised at how it works for her in terms of number of calls and bookings. Not all girls are concerned with the nitty gritties of esa and it works wonderfully without them putting in the amount of time and effort that most of us on the forum put in on a daily basis!
