The pleasure and pain centers in the brain are tightly intermingled. For many people those signals can get crossed and pain is interpreted as pleasure, and sometimes, vice versa. Likewise, what you or I would interpret as "extreme pain" their brain interprets as "extreme pleasure".
That or the OP is simply fucked up :)
